Abstract :
Purpoe To compare the bacteriologic and clinical efficacy of gatifloxacin and ciprofloxacin for the treatment of bacterial keratiti. Deign Propective, randomized clinical trial. Method A total of 104 eye of 104 patient with bacterial keratiti een at a tertiary eye-care center were randomized to treatment with gatifloxacin 0.3% eyedrop (GAT group, 50 eye) or ciprofloxacin 0.3% eyedrop (CIP group, 54 eye). Patient and the treating phyician were maked to the antibiotic being ued. Main outcome meaure tudied wa healing of the ulcer. Patient lot to follow-up before complete healing were excluded from further analyi. Reult A ignificantly higher proportion of ulcer in the GAT group exhibited complete healing compared with thoe in the CIP group (39 eye [95.1%] v 38 [80.9%]; P = .042). Gatifloxacin demontrated a ignificantly better action than ciprofloxacin againt gram-poitive cocci in vitro (P < .001), and the percentage of ulcer caued by thee pathogen that healed in the GAT group wa ignificantly better than in the CIP group (P = .009). Mean time taken for healing of ulcer and the efficacy againt gram-negative bacteria did not ignificantly differ between the two group. Concluion Gatifloxacin had a ignificantly better action againt gram-poitive cocci both in vitro and in vivo when compared with ciprofloxacin. In view of thee organim being the leading caue of keratiti worldwide, gatifloxacin may be a preferred alternative to ciprofloxacin a the firt-line monotherapy in bacterial keratiti.
