Abstract :
Purpoe Clinical and hitopathologic documentation of ympathetic ophthalmia (O) development in eye with potoperative bacterial endophthalmiti. Deign Obervational cae erie; retropective clinicopathologic tudy. Method All patient who preented with a clinical diagnoi of O during 2002 to 2004 were included in the tudy. The diagnoi of O wa made on the bai of hitory of penetrating ocular injury, followed by development of bilateral intraocular inflammation, ultraonographic detection of bilateral diffue thickening of the choroid, or both. Patient preenting with the additional finding of hypopyon underwent an anterior chamber tap and vitreou apirate for microbiologic detection of bacteria and fungi. Eight exciting eye were enucleated and ubmitted for hitologic examination. Reult Of a total of 26 patient with a clinical diagnoi of O, four alo had bacterial endophthalmiti. Of thee, hitologic examination of three exciting eye revealed vitreou abce and typical feature of O. Of the five remaining enucleated globe, hitologic examination howed that two eye had phacoanaphylactic endophthalmiti, and two other revealed feature of O; the one remaining eye had nongranulomatou diffue choroiditi. Concluion Bacterial endophthalmiti cannot prevent the development of O. Early diagnoi of coexitent mixed infectiou and inflammatory procee, and initiation of antimicrobial treatment directed at the infection followed by immunomodulatory agent to addre the autoimmune component may improve the prognoi in uch cae.
