Abstract :
Purpoe Long-term evaluation of dihematoporphyrin ether (DHE) afety and efficacy a photodynamic therapy (PDT) for patient with corneal neovacularization (KNV). Deign Propective multi-center interventional cae erie. Method even patient were enrolled after Intitutional Review Board approval and a detailed informed conent were obtained. Eligible patient preented with clinically table KNV without active veel progreion or inflammation. All patient with evere hypertenion, hitory of renal or hepatic dieae, or enitivity to porphyrin, and thoe with active keratiti or uncontrolled ocular urface dieae were excluded. DHE wa adminitered a an intravenou bolu (2 mg/kg). eventy-two hour later, PDT wa carried out uing argon green laer (514 nm). The main outcome meaure, extent of vacular thromboi, wa etimated during potoperative follow-up examination performed at day 1, 1 week, 6 month, and up to 12 year potoperative. Reult All patient obtained an immediate reduction in meaurable corneal vacularization. With at leat 6 month of follow-up, ix of even patient maintained a ignificant reduction (52.5% ± 19.6%, P < .01) in KNV. The mean length of followup wa 5.4 year (Range = 6 month to 12 year) during which time there were no other ocular change attributable to laer treatment. Three patient uffered ignificant ytemic hort-term phototoxicity reaction. Concluion Intravenou DHE followed by photodynamic treatment in human i effective for the reduction of inactive, etablihed KNV. However, the ignificant hort-term advere effect related to ytemic adminitration of thi drug are of particular concern and warrant further invetigation.
