  • Abstract
    Purpoe To evaluate tranpupillary thermotherapy (TTT) for the treatment of mall uveal melanoma of the poterior pole. Deign Propective, nonrandomized interventional cae erie. Method Eighteen patient underwent TTT for mall uveal melanoma located in the poterior pole of the eye. Tumor were between 2.5 and 4 mm in thickne. TTT wa performed with a diode laer at 810 nm. Patient had between one and three TTT eion, with an intenity adapted to the coloration of the fundu impact. Biomicrocopic examination, ultraonographic meaurement, and angiography were performed before and two month, four month, and ix month after treatment, then regularly during follow-up. Reult Eight of the 18 tumor regreed and 10 recurred. The one- and two-year metatai-free urvival rate calculated by the Kaplan-Meier method were, repectively, 61.11% to 44.44% (95% confidence interval). Recurrence were managed with enucleation (three patient), proton beam therapy (ix), or additional thermotherapy (one). After treatment, viual acuity wa maintained or improved for the eight patient with nonrecurrent tumor. Pathologic analyi of the three enucleated eye revealed cleral invaion. Concluion Depite encouraging initial hort-term reult obtained with TTT for the management of mall choroidal melanoma, the occurrence of evere complication, epecially recurrence and inufficient local tumor control, hould raie concern about indication for primary TTT given a iolated treatment for mall melanoma of the poterior pole.
