Abstract :
Purpoe To provide an update on mot recent development regarding ocular and ytemic manifetation and complication, clinical diagnoi and management, and molecular pathophyiology of peudoexfoliation (PEX) yndrome, and to dicu future tak and challenge in thi field. Deign Perpective. Method Review of recent literature and author’ own clinical and laboratory tudie. Reult PEX yndrome i a common age-related generalized fibrotic matrix proce of worldwide ignificance, which may not only caue evere chronic open-angle glaucoma and cataract, but alo a pectrum of other eriou pontaneou and urgical intraocular complication. Recent progre and advance have led to (1) improvement in clinical management by undertanding the effect of the PEX proce on ocular tiue, by refining diagnotic criteria, by applying new treatment regime, and by developing preventive trategie to reduce urgical complication; (2) increaing evidence for ytemic aociation of PEX with cardiovacular and cerebrovacular morbidity; and (3) new inight into the molecular pathophyiology by analyzing the compoition of PEX material, the differential gene expreion of affected tiue, and key factor involved in pathogenei. The current pathogenetic concept decribe PEX yndrome a an elatic microfibrillopathy involving tranforming growth factor-β1, oxidative tre, and impaired cellular protection mechanim a key pathogenetic factor. Concluion Future tak and challenge comprie epidemiologic prevalence and genetic tudie of PEX yndrome, propective randomized clinical and hitopathological creening tudie on it ytemic manifetation and aociation, and intenified baic reearch on differential protein and gene expreion, animal and in vitro model, a well a potential biomarker for PEX yndrome and it aociated glaucoma.
