Abstract :
Purpoe
To ae whether baeline retinal nerve fiber layer (RNFL) meaurement obtained with optical coherence tomography (OCT2; Carl Zei Meditec, Dublin, California, UA) are predictive of the development of glaucomatou change.
Deign
Cohort tudy.
Method
Participant were recruited from the Univerity of California, an Diego (UCD) longitudinal Diagnotic Innovation in Glaucoma tudy (DIG). One eye wa tudied from each of 114 glaucoma upect with normal tandard automated perimetry (AP) and OCT RNFL imaging at baeline. The cohort wa divided into two group baed on the development of glaucomatou change (repeatable abnormal viual field and/or a change in the tereophotographic appearance of the optic dik). Cox proportional hazard model were ued to determine the predictive ability of OCT RNFL thickne meaurement.
Reult
Over a 4.2-year average follow-up period, 23 eye (20%) developed glaucomatou change and 91 (80%) did not. At baeline, thinner RNFL meaurement, higher AP pattern tandard deviation (PD), “glaucoma” tereophotograph aement, and thinner central corneal thickne (CCT) were aociated with the tudy endpoint in univariate analyi. After adjuting for age, intraocular preure (IOP), CCT, and PD in multivariate model, a 10 μm thinner average, uperior and inferior RNFL at baeline wa predictive of glaucomatou change [hazard ratio (95% CI); 1.51 (1.11 to 2.12), 1.57 (1.17 to 2.18), and 1.49, (1.19 to 1.91), repectively]. Reult were conitent when tereophotographic aement wa included in multivariate analyi.
Concluion
Thinner OCT RNFL meaurement at baeline were aociated with development of glaucomatou change in glaucoma upect eye. RNFL thinning wa an independent predictor of the glaucomatou change, even when adjuting for tereophotograph aement, age, IOP, CCT, and PD.
