Purpoe
To meaure the actual doe of triamcinolone acetonide (TA) delivered during intravitreal injection performed by everal common technique.
Deign
Experimental tudy.
Method
A 0.1-ml, 40-mg vial of TA (Kenalog-40; Britol-Myer-quibb, Peapack, New Jerey, UA) wa prepared according to one of four protocol and the ma determined after drying overnight on waxed paper. In group 1, a 0.1-ml aliquot of TA wa dipened with a 30-gauge needle after haking the vial 10 or 30 time. Group 2 ued a 27-gauge needle. In group 3, the upernatant wa removed from the crytal. Group 4 paed the upenion through a 0.2-μm micropore filter and rined the crytal with aline.
Reult
There wa no tatitically ignificant difference between 30- or 27-gauge needle (P = 0.83, t tet) or between haking the vial 10 or 30 time before withdrawing the drug (P = 0.99). A tatitically ignificant difference (t tet, P = 0.001) wa found between TA delivered from the initial 60% of each yringe (mean ± D, 2.7 ± 1.0 mg) to that drawn from the lat 40% of each yringe (7.8 ± 3.6 mg). Group 3 had a mean weight of 32.1 ± 7.0 mg and group 4, 10.6 ± 2.1 mg.
Concluion
Effort to achieve a 4.0-mg doe of TA, regardle of method ued, are variable and inconitent. Injecting through a mall-gauge needle appear to concentrate the remaining upenion. Technique to concentrate TA or remove aqueou preervative by filtering effectively increae the concentration, but thee reult are variable.
