Trancleral Orbital Invaion by a Radiation and Chemotherapy-Reitant Choroidal Metatai of a Pulmonary Adenocarcinoma

Purpoe To report trancleral orbital invaion by a radiation- and chemotherapy-reitant choroidal metatai of a pulmonary adenocarcinoma. Deign Interventional cae report. Method A 51-year-old female wa found to have choroidal metatai from a previouly unknown primary pulmonary adenocarcinoma. he underwent external beam radiation treatment with a total doe of 40 Gray (Gy), delivered in 20 fraction over 28 day, followed by adjuvant chemotherapy with gemcitabine and carboplatin. After completing two of the three cheduled chemotherapy cycle, the choroidal leion wa oberved to have increaed in ize. Reult A tranvitreal apiration biopy of the choroidal leion confirmed the diagnoi of metatatic pulmonary adenocarcinoma. Enucleation wa performed for intractable pain. Hitopathology revealed invaion through cleral emiary channel to the retrobulbar tiue and through the optic nerve ubtance itelf. Concluion Depite appropriate external beam radiation therapy (EBRT) and chemotherapy, pulmonary adenocarcinoma metatae to the choroid can continue to grow and may invade the orbit tranclerally.
