Abstract :
Purpoe To decribe the clinical feature of an acute, inflammatory, and progreive retinal necroi that affect primarily the poterior pole. Deign Retropective, interventional cae erie. Method Twenty-even eye of 24 patient diagnoed with and treated for acute retinal necroi (ARN) were categorized into two group according to the predominant location of retiniti at preentation: either in the peripheral retina or in the poterior pole. Clinical feature, dieae progreion, viual outcome, and complication of thee two group were compared. Reult Fifteen eye demontrated the known peripheral retiniti pattern, and 12 eye exhibited a pattern of retiniti that affected mainly the poterior pole. Eye with peripheral retiniti howed focal, well-demarcated area of retinal necroi in the periphery with rapid circumferential progreion and rare involvement of the poterior pole. All eye with poterior pole retiniti had multifocal deep leion poterior to the vortex vein at preentation, and half of thee eye had leion in the macula. Thee leion progreed to patche of confluent retiniti in both the periphery and the poterior pole. There wa no ignificant difference between the two group in the incidence of anterior chamber and vitreou cell, vacular heathing, retinal hemorrhage, or optic dik edema. Patient with poterior retiniti involvement eemed to have a wore viual outcome during the firt two year after diagnoi. The Cox proportional hazard model uggeted a higher incidence of retinal detachment in patient with poterior retiniti (P = .07). Concluion The author report a pattern of herpetic retiniti that affect predominantly the poterior pole and may have a wore viual prognoi and a higher rate of retinal detachment.
