Macular chii and Detachment Aociated with Preumed Acquired Enlarged Optic Nerve Head Cup Original Reearch Article

Purpoe To decribe a clinical yndrome of macular chii and detachment in patient with acquired optic nerve head cupping reulting from glaucoma. Deign Retropective review of five patient. Method Patient were included if they had optic nerve cupping and macular chii with or without detachment with no other identifiable caue. The patient had to have no leakage on fluorecein angiography and no vitreou traction on examination or on optic coherence tomography (OCT). Thee patient were followed up and viual acuity, intraocular preure, and the finding of erial fundu and OCT examination were noted. Reult Five patient had chii with or without detachment of the macula with pronounced optic nerve head cupping. One patient had reolution of the macular fluid after filtering urgery for uncontrolled glaucoma. Two patient underwent a vitrectomy with intraocular ga and had almot total reolution of macular fluid and improved viion. Concluion Macular chii and detachment can occur in patient with preumed enlarged optic nerve head cup in the abence of obviou congenital anomalie of the dik. The author believe the caue i leakage of fluid from the vitreou through a tiny hole in the thin tiue of the cup. Thi i a imilar mechanim to that een in patient with optic pit. A vitrectomy or tep to reduce the intraocular preure may reult in reolution of the fluid and improved viion.
