Abstract :
Purpoe To evaluate the frequency of reopening of macular hole after cataract extraction. Deign Retropective, comparative, conecutive cae erie. Method Two hundred and eleven eye with idiopathic macular hole cloed by vitrectomy were divided into four group: Group 1: prior cataract extraction; Group 2: vitrectomy then cataract extraction; Group 3: vitrectomy only; and Group 4: vitrectomy and cataract extraction a a combined procedure. The main outcome meaure of macular hole reopening wa evaluated in relationhip to multiple variable. Reult Two hundred and eleven eye were included: Group 1: 56 eye; Group 2: 86 eye; Group 3: 41 eye; and Group 4: 28 eye. Twenty-four macular hole reopened (11%) (mean follow-up 26.6 month, range, three to 118 month). The greatet number of macular hole reopening, 17 (20%), were in Group 2. Cox multivariate analyi failed to demontrate an aociation between duration of hole, erum ue, internal limiting membrane peeling, or tage and reopening of a macular hole. Cox analyi howed a four-fold increaed rik of reopening in Group 2 eye (95% confidence interval [CI]: 1.7 to 11.2; P = .002). Eye with cytoid macular edema after cataract extraction had a even-fold increaed rik of macular hole reopening (7.72; 95% CI: 2.79 to 21.3; P < .0005). Kaplan-Meier analyi howed increaed rate of macular hole reopening in Group 2 eye compared to the other 3 group combined (log-rank P < .00005). Concluion Cataract extraction after ucceful vitrectomy for macular hole, when complicated by cytoid macular edema (CME), may increae the rik of macular hole reopening.
