Abstract :
Purpoe To evaluate the change in viual acuity and retinal appearance in patient after early initiation of intravitreal bevacizumab treatment for central retinal vein occluion (CRVO). Deign Retropective, interventional cae erie. Method Patient with CRVO of fewer than three month’ duration receiving intravitreal bevacizumab a primary treatment were evaluated. Patient received an intravitreal 1.25 mg (0.05 ml) bevacizumab injection. Change in viual acuity, central macular thickne, venou tortuoity and diameter, and optic dik edema were noted. Reult ix eye of five conecutive patient with CRVO treated with intravitreal bevacizumab injection were reviewed retropectively. The patient did not have other ocular condition that could have compromied viual acuity. The mean baeline viual acuity wa 20/428 (logarithm of the minimum angle of reolution [logMAR] unit, 1.33). The mean follow-up period wa 12 month (range, even to 15 month), and the number of bevacizumab injection ranged from four to 10. The patient howed a tatitically ignificant decreae in optic nerve head welling, venou tortuoity, and venou diameter, with the larget proportion of change occurring within one month of the firt bevacizumab injection. The mean viual acuity at lat follow-up wa 20/53 (logMAR unit, 0.42; P = .035, a compared with baeline). In no patient did collateral veel at the optic nerve head develop. Concluion The patient experienced a dramatic improvement in the viual acuity and clinical fundu appearance, without collateral veel formation. Thee finding are difficult to explain with current theorie of the pathophyiologic feature of CRVO. Thee finding alo ugget early initiation of anti–vacular endothelial growth factor (VEGF) treatment hould be tudied in a larger trial for CRVO.
