Abstract :
Purpoe To evaluate the long-term efficacy of intraocular preure (IOP) reduction and complication of Ahmed glaucoma valve (AGV) implantation (New World Medical, Inc, Rancho Cucamonga, California, UA) in refractory glaucoma. Deign Retropective cohort tudy. Method Retropective medical record from 64 patient (78 eye) with refractory glaucoma who underwent AGV implantation with a minimum of three year of follow-up were reviewed. Data regarding age, gender, race, eye laterality, pecific glaucoma diagnoi, bet-corrected viual acuity (BCVA), number of medication, IOP, viual field, urgical complication, and follow-up interval were collected from all viit and were analyzed. The primary outcome meaure wa cumulative probability of ucce defined a IOP of le than 21 mm Hg and of 5 mm Hg or more with a minimum of 15% reduction from baeline IOP, without additional glaucoma urgery or lo of light perception. econdary outcome included IOP and number of medication at three, ix, 12, 24, 36, 48, and 60 month after urgery, urgical complication, and final BCVA. Reult The cumulative probability of ucce wa 80% and 49% at one and five year, repectively. IOP wa reduced from a mean of 30.4 ± 10.7 mm Hg to 17.0 ± 5.0 mm Hg at 12 month and 15.9 ± 3.0 mm Hg at 60 month (P < .001). The number of medication decreaed from 3.2 ± 1.0 medication at baeline to 1.6 ± 0.4 at 12 month and 2.1 ± 0.2 at 60 month (P < .001). Prior glaucoma urgery and the ilicone type of AGV were tatitically ignificant rik factor for failure (P < .001). Concluion Approximately 50% of ingle-plate AGV implantation in refractory glaucoma were conidered ucceful after five year of follow-up. Prior glaucoma urgery wa a tatitically ignificant rik factor for failure.
