Abstract :
Purpoe To determine the clinical value of erological teting in patient with idiopathic cleriti. Deign Retropective cae erie. Method Medical record of patient with cleriti een at an intitutional referral center over an 11-year period were reviewed. Reult Of 119 patient with cleriti een at the Univerity of Illinoi Uveiti Clinic, 91 (76.5%) patient had no known etiology at initial preentation. eventy of the 91 patient were teted for rheumatoid factor (RF), 19 (27.1%) of whom had a poitive reult. Ten (52.6%) of thee RF poitive patient were ubequently diagnoed with rheumatoid arthriti (RA) during a mean follow-up of 10.6 month (range, zero to 72 month), wherea only one of 51 (2.0%) RF negative patient developed RA, producing an odd ratio for developing RA in RF poitive patient of 55.6 (95% confidence interval (CI) 7.8 to 369.8, P = .00001). Of the 70 patient who were teted for anti-neutrophil cytoplamic antibody (ANCA), even (10.0%) teted poitive. Three (42.9%) of the ANCA poitive patient ubequently developed Wegener granulomatoi (WG), wherea only two of 63 ANCA negative patient (3.2%) developed WG during a mean follow-up of 8.4 month (range, zero to 72 month). The odd ratio for developing WG in patient with idiopathic cleriti and a poitive ANCA creen compared with a negative ANCA wa 22.9 (95% CI 3.4 to 154.2, P = .006). Concluion The likelihood of patient with idiopathic cleriti developing RA and WG wa increaed if they had a poitive RF or ANCA, upporting the role of immunologic marker teting in patient who preent without ytemic dieae.
