Abstract :
Purpoe To invetigate the aociation of clinically ignificant macular edema (CME) and long-term urvival in individual with type 1 and type 2 diabete. Deign Population-baed cohort tudy. Method The Wiconin Epidemiologic tudy of Diabetic Retinopathy (WEDR) i an ongoing propective population-baed cohort tudy initiated from Augut 21, 1980 through July 30, 1982 of individual with diabete diagnoed at either younger than 30 year of age (younger-onet group; n = 996) or 30 year of age or older (older-onet group; n = 1,370). tereocopic color retinal photograph were graded for retinopathy uing the modified Airlie Houe claification cheme. CME wa defined by the Early Treatment Diabetic Retinopathy tudy criteria. Reult Prevalence of CME wa 5.9% and 7.5% for the younger- and older-onet group, repectively. After 20 year of follow-up, 276 younger-onet and 1,197 older-onet peron died. When adjuting for age and gender, CME wa not ignificantly aociated with all-caue mortality (hazard ratio [HR], 1.41; 95% confidence interval [CI], 0.96 to 2.07; P = .08) or ichemic heart dieae mortality (HR, 1.14; 95% CI, 0.61 to 2.12; P = .68) in the younger-onet group. In the older-onet group, there wa increaed all-caue and ichemic heart dieae mortality when CME wa preent (HR, 1.55; 95% CI, 1.25 to 1.92; P < .01; and HR, 1.56; 95% CI, 1.15 to 2.13; P < .01, repectively), when adjuting for age and gender. After controlling for other rik factor, the aociation remained ignificant for ichemic heart dieae (HR, 1.58; 95% CI, 1.07 to 2.35; P = .02) among thoe taking inulin. CME wa not ignificantly aociated with troke mortality in either group. Concluion CME eem to be a rik indicator for decreaed urvival in peron with older-onet diabete mellitu. The preence of CME may identify individual who hould be receiving care for detection and treatment of cardiovacular dieae.
