Abstract :
Purpoe
To determine the incidence of and rik factor for hemorrhagic complication in patient on anticoagulation (ACT) or antiplatelet therapy (APT) having glaucoma urgery.
Deign
Retropective cae-control tudy.
Method
Medical record of patient who had glaucoma urgery between July 1, 1998 and March 31, 2005 were reviewed. Patient who either ued ACT/APT continuouly throughout the perioperative period or dicontinued it ue prior to urgery were compared to cae-matched control patient who were not on uch therapie. Patient on ACT/APT who experienced potoperative hemorrhagic complication were compared to thoe who did not. Outcome meaure included hemorrhagic complication and thromboembolic event.
Reult
Three hundred and forty-even patient (eye) who were on ACT or APT prior to glaucoma urgery had a higher rate of hemorrhagic complication than 347 control patient (10.1% v 3.7%, repectively, P = .002). Patient on ACT had a higher rate of hemorrhagic complication than patient on APT (22.9% v 8.0%, repectively, P = .003). Patient who continued ACT during glaucoma urgery had the highet rate of hemorrhagic complication (31.8%) when compared to patient who dicontinued ACT prior to urgery or patient who ued APT alone (P = .001). Hemorrhagic complication following glaucoma urgery were more frequently aociated with preoperative ACT, arrhythmia, and higher preoperative and potoperative intraocular preure (IOP).
Concluion
Chronic ACT/APT wa aociated with a tatitically ignificant increae in the rate of hemorrhagic complication, and perioperative ACT and a high preoperative IOP are potential rik factor for hemorrhagic complication in patient undergoing glaucoma urgery.
