Abstract :
Purpoe
The effect of increaed corneal thickne after Decemet tripping endothelial keratoplaty (DEK) on intraocular preure (IOP) meaurement ha not been previouly tudied. It i uncertain if thi increae in corneal thickne would artificially elevate IOP reading by Goldmann tonometry [GAT] (Haag-treit, Konig, witzerland). Therefore the effect of DEK-related thick cornea on IOP meaurement wa invetigated uing three different technique.
Deign
Propective cro-ectional tudy.
Method
Participant were recruited from a ingle tertiary referral center. Fifty eye of 38 patient with ucceful DEK at leat three month prior to teting were evaluated. At the time of the tudy, none of the participant had clinically detectable corneal edema. IOP wa meaured with GAT, pneumatonometry, and dynamic contour tonometry (DCT) in an unmaked randomized equence. Central corneal thickne (CCT) wa meaured by ultraonic pachymetry.
Reult
Mean CCT wa 701 ± 68 μm. The mean IOP ± tandard deviation (D) wa 15.9 ± 4.9 mm Hg for GAT, 20.3 ± 4.5 mm Hg for pneumatonometry, and 19.8 ± 4.4 mm Hg for DCT. Pneumatonometry and DCT IOP meaurement were ignificant higher than GAT (P < .01). In contrat, the difference between pneumatonometry and DCT reading wa not tatitically ignificant (P = .28). The correlation between IOP and corneal thickne were not ignificant in thi cohort (P > .05).
Concluion
Falely elevated GAT, a expected in thick cornea, wa not demontrated after DEK. High IOP reading by GAT therefore hould raie upicion of elevated IOP in DEK eye.
