Abstract :
Purpoe
To explore the utility of keratometry and central corneal thickne (CCT) meaurement in diagnoi of Marfan yndrome.
Deign
Retropective chart review of 211 patient referred for ocular examination to rule out Marfan yndrome.
Method
Patient were categorized into Marfan or control group uing current clinical diagnotic criteria. Patient in whom Marfan yndrome could be neither diagnoed nor ruled out were categorized a inconcluive. Thoe with other connective tiue dieae were excluded from the tudy. T tet were performed on keratometry and CCT value, and odd ratio (OR) were calculated.
Reult
ixty-two patient were in the Marfan group and 98 patient were in the control group. Mean age of the Marfan group wa 22.3 year v 19.3 year in the control group (P = .23). Marfan patient had ignificantly lower keratometry and CCT value than control (40.8 diopter (D) [Marfan] v 43.3 D [control], P = 4.0 × 10−19; and 543.5 μm [Marfan] v 564.2 μm [control], P = 9.1 × 10−6). The maximum OR for keratometry (17.6) wa calculated at 42 D. In Marfan yndrome patient without ectopia lenti, thee value were 41.5 D (P = .00026 v control) and 542.0 μm (P = .0011 v control).
Concluion
There wa a highly ignificant difference in keratometry value between Marfan and control patient, and value le than 42 D could be ued a a clinical diagnotic criterion for Marfan yndrome. ignificant overlap in CCT value between Marfan and control patient ugget that further invetigation i neceary to determine the clinical utility of CCT meaurement.
