Abstract :
Purpoe
To report the anatomic and functional reult of primary vitrectomy without cleral buckling for the treatment of peudophakic rhegmatogenou retinal detachment (PRD).
Deign
Propective, nonrandomized urgical technique tudy.
Method
One hundred eye of 98 patient with PRD were operated by vitrectomy alone. Internal ubretinal fluid drainage, cryocoagulation and/or endolaer and fluid–air exchange with ulfur hexafluoride 20% wa applied in all cae. The preoperative and potoperative characteritic were analyzed. Main outcome meaure were anatomic ucce rate after initial urgical intervention and after reoperation for primary failure, viual outcome at the lat follow-up viit, and complication.
Reult
Mean follow-up ± tandard deviation (D) wa 12 ± 6.3 month (range, even to 36 month). Mean final viual acuity ± D wa 0.42 ± 0.45 logarithm of the minimum angle of reolution (logMAR) compared with 0.95 ± 0.73 logMAR before urgery (P < .01). Mean number ± D of retinal break found before urgery wa 1.36 ± 1.12 (range, zero to five), and an additional 1.58 ± 2.26 (range, zero to 15) retinal break were found during urgery. The retina wa reattached uccefully after a ingle urgery in 92 eye (92%). Recurrence of retinal detachment occurred in eight eye (8%), caued by proliferative vitreoretinopathy in ix eye (75%) and by new break in two eye (25%). Final anatomic reattachment wa obtained in thee cae after a mean of 1.75 ubequent operation. Three eye required permanent ilicone oil tamponade o that final anatomic ucce wa achieved in 97 eye (97%). The mot common potoperative complication wa ocular hypertonia of more than 21 mm Hg, oberved in 36 (36%) eye, which wa managed uccefully.
Concluion
Primary vitrectomy without cleral buckling provide a high anatomic ucce rate in eye with PRD and i aociated with few complication.
