Abstract :
Purpoe To evaluate the change in the pilocarpine-induced contractility of the ciliary mucle in eye with prebyopia before and after cataract extraction uing ultraound biomicrocopy (UBM). Deign Propective interventional cae erie. Method A clear corneal phacoemulification and poterior chamber intraocular len (Acryof A60AT; Alcon Laboratorie, Fort Worth, Texa, UA) wa implanted in 23 eye in 15 ubject. UBM wa performed with and without intilling 2% pilocarpine, a well a before and two month after cataract extraction. The mean (± tandard deviation) age wa 65.33 ± 6.09 year (range, 56 to 75 year). The increae in the axial length of the ciliary body (CBAXL) wa ued a a urrogate indicator of the centripetal ciliary mucle contraction during accommodation. Image of the ciliary body were compared viually uing Adobe Photohop 7.0 (Adobe ytem Inc, an Joe, California, UA). Reult The CBAXL value with and without pilocarpine before cataract extraction wa 1.708 ± 0.165 mm and 1.689 ± 0.187 mm, repectively, which wa not ignificantly different (P = .261). The CBAXL value with and without pilocarpine after cataract extraction wa 1.998 ± 0.375 mm and 1.675 ± 0.279 mm, repectively, which wa ignificantly different (P < .001). The viually compared configuration of the change in the ciliary body were compatible with the analyi of the meaured parameter. Concluion Pilocarpine induced only ubtle movement of the ciliary body before cataract urgery. However, after cataract extraction, it induced ignificant centripetal movement of the ciliary body compared with that without pilocarpine. Thi how that a lenticular clerotic component may influence both len movement and the contractility of the ciliary mucle, and i believed to be related to the prebyopia.
