Abstract :
Purpoe To determine whether eye bank prediected corneal graft provide outcome comparable to urgeon-diected graft for Decemet tripping automated endothelial keratoplaty (DAEK). Deign Randomized, propective, double-maked clinical trial. Method Twenty pair of donor cornea were harveted. One cornea from each pair wa randomized to be precut at an eye bank for next-day ue. The urgeon diected the fellow cornea intraoperatively uing a comparable microkeratome and protocol. The cornea were randomly aigned to 40 ubject having DAEK at a ingle center. ubject and evaluator were maked and tatitical ignificance wa aeed uing the paired t tet. Reult Mean ubject age wa 71 ± 12 year and 90% had Fuch dytrophy. Mean endothelial cell lo wa 32% at ix month and 34% at one year; the two group did not differ by a tatitically ignificant amount at either time point (P = .10 and P = .79, repectively). Each group experienced two early dilocation (10%), and graft were repoitioned uccefully with a econd air bubble. At ix month, 28 of 35 patient (80%) had bet-corrected viion of 20/40 or better, excluding five patient (12%) with preexiting retinal problem (P = .48). Both group experienced a mild hyperopic hift (P = .82), and neither had a tatitically ignificant increae in mean refractive cylinder (P = .63). Hitology from one ubjectʹ eye potmortem demontrated that endothelial cell had migrated over the expoed edge of the donor troma a year after urgery. Concluion Eye bank precut tiue provided imilar endothelial cell lo, viual and refractive outcome, and detachment rate compared with urgeon-diected tiue.
