Abstract :
Purpoe To invetigate the development of typical age-related macular degeneration (AMD) and polypoidal choroidal vaculopathy (PCV) in fellow eye of Japanee patient with exudative AMD. Deign Retropective obervational conecutive cae erie. Method Two hundred and ixteen Japanee patient were enrolled in thi tudy from the outpatient clinic of the Univerity of Tokyo Hopital. Ninety-one patient had typical AMD and one hundred and twenty-five patient had PCV. The average follow-up period wa 33.6 and 25.1 month for typical AMD and PCV patient. Reult The cumulative incidence of involvement in fellow eye with overall exudative AMD, including both typical AMD and PCV, wa 3.4% in one year, 9.3% in three year, and 11.3% in five year. It wa 3.6%, 7.3%, and 11.2% in typical AMD, and 3.2%, 11.1%, and 11.1% in PCV in one, three, and five year, repectively. Before the development of exudative AMD, patient with typical AMD had a variety of funducopic finding including retinal pigment epithelium (RPE) atrophy, druen, druenoid pigment epithelial detachment (PED), and normal macula. PCV patient, on the other hand, had funducopic finding of RPE atrophy. Inner choroidal vacular abnormality of vacular network and polypoidal formation wa oberved in everal eye before the clinical manifetation of exudative change. Concluion Typical AMD and PCV had imilar probabilitie of involving the fellow eye in unilaterally affected Japanee patient. RPE atrophy wa a prevailing finding in fellow eye of patient who developed PCV. In PCV, choroidal vacular network and polypoidal formation gradually grow before exudative change.
