Abstract :
Purpoe
To ae the incidence of vitreomacular adheion and traction in age-related macular degeneration (AMD), and to evaluate urgical treatment in a ubet of patient with choroidal neovacularization (CNV) nonreponive to anti-neovacular growth factor (anti-VEGF) treatment.
Deign
Retropective obervational cae-control and interventional cae erie.
Method
pectral optical coherence tomography, combined with imultaneou canning laer ophthalmocope (pectral OCT/LO), wa performed in 170 eye of 94 elderly patient, 61 with exudative AMD, 59 with nonexudative AMD, and 50 control eye. The preence of hyaloid adheion to the poterior pole, and vitreomacular traction (VMT) were determined. Five patient with VMT underwent urgical hyaloid removal. Bet-corrected viual acuity (BCVA) and retinal thickne were evaluated a outcome.
Reult
Hyaloid adheion wa preent in 17 eye with exudative AMD (27.8%), 15 eye with nonexudative AMD (25.4%), and eight control eye (16%). ignificant difference wa found among the group (P = .002). Among the eye with hyaloid adheion, VMT wa hown in 10 eye (59%) with exudative AMD, two eye (13%) with nonexudative AMD, and one control eye (12%). VMT wa aociated with the everity of AMD (P = .0082). The area of hyaloid adheion wa ignificantly maller than and concentric to the area of CNV complex in eye with exudative AMD. Eye with VMT that underwent urgery experienced a modet improvement of BCVA and decreae of retinal thickne.
Concluion
Hyaloid adheion to the macula i aociated with AMD, and frequently caue VMT in eye with CNV. Tractional force may antagonize the effect of anti-VEGF treatment, and caue pharmacological reitance in a ubpopulation of patient. Future tudie are needed to define the role of vitreoretinal urgery in uch cae. pectral OCT/LO allow careful diagnoi and follow-up.
