Abstract :
Purpoe To evaluate long-term outcome after Ahmed valve implantation in patient with glaucoma when uing adjunctive intraoperative mitomycin C (MMC) and potoperative 5-fluorouracil (5-FU). Deign Retropective, interventional, conecutive cae erie. Method A conecutive erie of eye undergoing Ahmed valve implantation, either alone (AHMED eye) or in combination with cataract urgery (AHMED+PHACO), uing both intraoperative MMC and potoperative 5-FU were evaluated. Failure wa defined a the firt occurrence of any of the following: 1) the firt of three conecutive viit where intraocular preure (IOP) wa >18 mm Hg or <20% IOP reduction from baeline and the final number of topical medication wa not reduced by at leat two from baeline, 2) the need for additional urgery, or 3) the development of eriou complication. Reult A total of 130 eye underwent Ahmed valve implantation with intraoperative expoure to 0.5 mg/ml MMC (median time: eight minute; range, four to 10) and potoperative ubconjunctival injection of 5 mg of 5-FU (median: five injection; range, zero to nine). Kaplan-Meier etimate of the cumulative probability of valve ucce and confidence interval (CI) at the ixth follow-up year were 0.72 (95% CI, 0.59 to 0.82) for AHMED eye (n = 88), 0.84 (95% CI, 0.65 to 0.93) for AHMED+PHACO eye (n = 42). A median of two fewer medication were required relative to baeline for both AHMED and AHMED+PHACO eye. Concluion The adjunctive ue of both intraoperative MMC and potoperative 5-FU with Ahmed valve implantation reult in high ucce rate. IOP wa well controlled in the majority of patient within the ix-year potoperative period.
