Abstract :
Purpoe To evaluate whether adding perioperative topical ketorolac tromethamine 0.4% improve cataract urgery outcome relative to topical teroid alone in patient without known rik factor for cytoid macular edema (CME). Deign Propective, randomized, invetigator-maked, multicenter clinical trial. Method Patient cheduled to undergo phacoemulification and with no recognized CME rik (diabetic retinopathy, retinal vacular dieae, or macular abnormality) were randomized to receive either predniolone acetate 1% 4 time daily (QID) alone (teroid group; n = 278) or predniolone 1% QID plu ketorolac 0.4% QID (ketorolac/teroid group; n = 268) for approximately four week potoperatively. In the ketorolac/teroid group, patient alo received topical ketorolac 0.4% QID for three day preoperatively. In both group, patient received four doe of ketorolac 0.4% one hour before urgery. Patient with capular diruption or vitreou lo intraoperatively were exited from the tudy. Outcome meaure included CME incidence, retinal thickne a meaured by optical coherence tomography (OCT), bet-corrected viual acuity, and contrat enitivity. Reult No patient in the ketorolac/teroid group and five patient in the teroid group had clinically apparent CME (P = .032). Baed on OCT, no ketorolac/teroid patient had definite or probable CME, compared with ix teroid patient (2.4%; P = .018). In the ketorolac/teroid group, mean retinal thickening wa le (3.9 μm v 9.6 μm; P = .003), and fewer patient had retinal thickening of more than 10 μm a compared with the teroid group (26% v 51%; P < .001). Concluion Thi tudy ugget that adding perioperative ketorolac to potoperative predniolone ignificantly reduce the incidence of CME and macular thickening in cataract urgery patient already at low rik for thi condition.
