Abstract :
Purpoe
To ae the role of anti-tubercular therapy in uveiti with latent/manifet tuberculoi (TB).
Deign
Retropective, interventional cae erie.
Method
A total of 360 patient from uveiti clinic with following incluion criteria were tudied: 1) complete clinical record of viual acuity, lit-lamp biomicrocopic examination, intraocular preure, complication if any, and treatment record at the baeline and at all follow-up viit; 2) a documented poitive tuberculin kin tet (10 mm of induration or more) at 48 to 72 hour; 3) evidence of active uveiti, i.e., cellular reaction in the anterior chamber with or without keratic precipitate, and/or active vitreou inflammation, retinal vaculiti, choroiditi, or neuroretiniti; 4) all known caue of infectiou uveiti except TB and known noninfectiou uveitic yndrome ruled out; and 5) a minimum one year of follow-up from the initiation of treatment. Of thee, 216 patient (Group A) received four-drug anti-tubercular therapy and corticoteroid, and 144 patient (Group B) received corticoteroid alone. The main outcome meaure wa recurrence of inflammation after minimum ix month of initiating treatment in each group.
Reult
Recurrence reduced ignificantly (P < .001) in Group A (15.74%) a compared to Group B (46.53%) over a median follow-up of 24 and 31 month, repectively. The patient treated with anti-tubercular therapy with corticoteroid had decreaed rik of developing recurrence of uveiti by approximately two-third a compared to thoe treated with corticoteroid alone.
Concluion
Addition of anti-tubercular therapy to corticoteroid in uveiti patient with latent/manifet TB led to ignificant reduction in recurrence of uveiti.
