Abstract :
Purpoe To compare the viual outcome following deep anterior lamellar keratoplaty (DALK), penetrating keratoplaty (PK), and manual top-hat PK (TH-PK) in ubject undergoing corneal tranplantation for keratoconu (KC). Deign A retropective comparative cae erie. Method etting: Cornea clinic at the Toronto Wetern Hopital. tudy population: Fifty patient who underwent corneal tranplantation for KC: 17 eye underwent DALK, 20 eye underwent traditional PK, and 13 had TH-PK. main outcome meaure: Preoperative and potoperative uncorrected and bet pectacle-corrected viual acuity (UCVA, BCVA), high-order aberration (HOA), complication rate, and endothelial cell count. Reult The median BCVA at 12 month follow-up wa 20/40 in the DALK eye and 20/30 in the traditional PK and TH-PK eye. The mean final pherical equivalent power in the three group wa le than −1 diopter (D). The median atigmatim wa le than 3.5 D in the three group. Complication rate were imilar for three group, although the DALK group tended toward more complication. Although DALK and TH-PK procedure reult in ignificantly horter time to uture removal (P < .01), they caued increaed level of HOA (P = .02). Endothelial cell count at 12 month were ignificantly higher in DALK and TH-PK eye when compared to the traditional PK eye (P < .001). Concluion DALK, PK, and TH-PK provide comparable viual outcome in keratoconu patient. Although DALK and TH-PK induce more HOA, they peed up the time to uture removal and provide higher endothelial cell denity at one year of follow-up.
