Abstract :
Purpoe
To determine the effect of corneal light catter on viion after penetrating keratoplaty (PK).
Deign
Cro-ectional tudy.
Method
etting: Cornea ervice at Mayo Clinic, Rocheter, Minneota. tudy population: ixty-one eye with clear graft at 14.6 ± 10.5 year after PK; 12 eye with late endothelial failure (LEF) at 19.7 ± 6.4 year after PK; 41 normal eye. obervation procedure: PK. main outcome meaure: Backcatter from different depth of the cornea wa meaured by a cutom catterometer; intraocular forward light catter wa meaured with a tray light meter; high-contrat viual acuity (HCVA) wa meaured by Early Treatment Diabetic Retinopathy tudy (ETDR) or nellen chart; meopic and photopic low-contrat viual acuity (LCVA) were meaured with a 10% contrat chart.
Reult
Backcatter from the anterior, middle (troma), and poterior third of the cornea wa higher in clear graft compared with normal (P < .001), and higher in graft with LEF compared with clear graft (P < .001). Corneal tromal backcatter from clear graft correlated with time after keratoplaty (r = 0.21; P = .006; n = 61). Forward light catter wa higher in eye with clear graft compared with normal (P ≤ .006), and higher in eye with LEF compared with clear graft (P ≤ .01). Corneal tromal backcatter from clear graft correlated with HCVA (r = 0.34; P = .02; n = 52), meopic LCVA (r = 0.36; P = .01; n = 28), and photopic LCVA (r = 0.57; P < .001; n = 28). Forward catter in eye with clear graft correlated with photopic LCVA (r = 0.38; P = .04; n = 26).
Concluion
Light cattered from the cornea increae with time after PK and i aociated with decreaed high- and low-contrat viion.
