Abstract :
Purpoe To report four cae of hypotropic diociated vertical deviation (DVD) and to decribe the clinical feature of thi rare diorder accompanied by a literature review. Deign Interventional cae erie and literature review. Method Four conecutive cae of hypotropic DVD and reult of a literature review are preented. Data regarding age, viual acuity, laterality, amount of deviation, and type of urgery were analyzed. Reult Collating data from previou report with thi cae erie identified a total of nine cae of DVD. Mean age at diagnoi wa 26.1 year (range, two to 52 year). All except one patient had unilateral DVD. The average amount of hypotropia wa 25.4 prim diopter. All but one patient had evere monocular viion deficit ranging from 6/16 to hand movement. The remaining patient had relatively good viion (6/7.5) and tereopi (50 econd of arc). Five patient had high myopia, and in three patient, the DVD wa related to penetrating ocular injury. No cae of DVD were aociated with congenital trabimu. A large receion of the inferior rectu mucle wa performed in three patient and a combined receion-reection of the mucle wa performed in another three patient. The reult of both type of urgery were atifactory. Concluion Hypotropic DVD i motly unilateral and commonly i aociated with monocular viual deficit or high myopia. Although the nature of the intermittent low downward ocular deviation i imilar to that of hypertropic DVD, it hould be conidered to be a unique form of the diociated trabimu complex. Thi rare condition can be corrected urgically by a large receion or a combined receion-reection of the inferior rectu mucle.
