Traumatic rupture of the diaphragm (TRD) poses a challenge to bot h rad iologist s and surgeons. They are uncommon and occur following blunt abdominal or lower thoracic traum a. The right side involveme nt is less common than the left side and is easily missed . Spiral computed tomograph y (Spiral CT) with image reformat ion is very useful in the diagnosis of TRD and in identifying associated injur ies. Early diagn osis and rep air redu ces mor tality and morb idity. We present the case of a 16 year old boy who was involved in a high speed tra ffic accide nt with blunt injury to his th or ax and abdomen. He was referred from a pe riphe ra l hospit al in Oman for fur the r man agement at Sultan Qaboos University Hospit al. A spiral CT scan of th or ax and abdomen with image reformation helped in th e early diagno sis and man agement of the t raumatic rupture of his righ t hemidiaphragm.
