In this paper we report on the photonic realization of two-qubit, polarization and spatially-encoded single photon, quantum Controlled Hadamard gate in Titanium diffused LiNbO3 channel waveguides, using continuous wave pump source with λ = 0.812µm wavelength. In this design, sensitivity to TM-polarization defines the controlling behavior of the designed circuit, and the computed Electrical-field amplitudes in the output branches, verifies its quantum Hadamard functionality. All the simulations in this paper were carried out using numerical beam propagation method incorporated in RSoft program package .
