FACTORS AFFECTING LENGTH OF STAY IN POSTOPERATIVE CARE UNIT FOLLOWING GYNECOLOGICAL OPERATIONS

Abstract :
Aim: This retrospective study aimed to investigate postoperative care unit (PACU) length of stay and factors influencing this stay following elective gynecological operations.Material and Methods: The anesthesia and PACU records of patients between ages of 18-80 years undergoing gynecological laparotomy with surgical time ≥ 60 minutes under routine general anesthesia and postoperative analgesia were included in the study. The effects of age, body mass index, operation duration, physical status according to classification system of American Society of Anesthesiologist (ASA), pathological diagnosis of operation, intraoperative fluid and anesthetic consumption, PACU entry Aldrete score and treatments administered in PACU on primary outcome parameter- namely PACU length of stay categorized into 60 min and ≥60 min- were studied.Results: The factors predicting PACU stay ≥ 60 minutes were found to be ASA status ≥ III, malignant pathology, PACU entry score 9, respiratory predicting PACU stay ≥ 60 minutes were found to be ASA status ≥ III, malignant pathology, PACU entry score 9, respiratory physiotherapy and invasive mechanical ventilation requirement with Odds Ratio of 3.27, 2.38, 3.28, 3.37 and 14.6 respectively. Conclusion: When planning PACU occupancy for gynecological patients, ASA status ≥ III, malignant pathology, PACU entry Aldrete score 9, respiratory physiotherapy and invasive mechanical ventilation requirement can be considered as a predictor for prolonged PACU stay ≥ 60 minutes.
