Pre-eruptive intracoronal resorption and its management: case report

INTRODUCTION: Pre-eruptive intra-coronal resorption (PEIR) is a defect located in the dentin of an unerupted tooth, just beneath the dentin-enamel junction. The prevalence of PEIR is 3-6% in patients and 0.5-2% in teeth. In some case reports, it is suggested that the unerupted tooth should be surgically exposed, and the lesion should be restored to prevent the further destruction of the coronal structure. The aim of this case report was to describe the management of a mandibular permanent molar tooth with PEIR. CASE REPORT: A 6-year-old boy patient was referred to the Pediatric Dentistry Clinic with a complaint of pain, gingival redness, and an unerupted tooth. In the clinical examination, it was observed that the eruption sequence was compatible with his age; however, the right mandibular first permanent molar was absent. Under inhalation sedation, the gingival tissue above the unerupted molar was surgically removed with a diode laser (SIROLaser Xtend, Bensheim, Germany). The soft tissue into the lesion was carefully removed with an excavator. A sample of the removed tissue was evaluated histopathologically. A glass ionomer cement (GC Fuji XI GP, Tokyo, Japan) was placed as a base, and the restoration was completed with composite resin (3M ESPE, Seefeld, Germany). After a follow-up period of 16 months, no clinical symptoms were observed, and the development of the root was detected to continue. CONCLUSION: Early diagnosis and treatment of PEIR is the main objective for the development of the tooth and maintenance of pulpal health.
