Abstract :
Objective: The aim of this study is to comparethe sensitivity of MRI, clinical examination andstandard x-ray of sacroiliac joints and lumbar spine inspondyloarthrites.Methods: In a retrospective study, patients withspondyloarthritis were included according to AMORcriteria, in the 2003-2015 period. All patients hadstandards x-rays and MRI examination of the sacroiliacjoints and/or the lumbar spine. The radiographs andMR scans were evaluated by a senior rheumatogist,according to MRI LEEDS scoring system, SASSscore and modified New York criteria for ankylosingspondylitis.Results: We included 47 patients. There was noconcordance between the clinical examination andthe sacroiliitis to the standard x-rays (Chi-2 : 3.072 ;Kappa : 0.26 ; p : 0.08) or to the MRI exploration ofsacroiliac joints (Chi-2 : 0.840 ; Kappa : -0.14 ; p : 0.36),and between sacroiliitis to the standard x-rays and tothe MRI (Chi-2 : 1.493 ; Kappa : 0.19 ; p : 0.22). Among 13patients with non-radiographic axial spondyloarthrites,MRI has objectified inflammatory sacroiliitis in 6cases (46.2%). There was no concordance betweenstandard x-rays and MRI exploration in detection ofinflammatory Romanus lesions at lumbar spine (Chi-2 :2.62 ; Kappa : 0.23 ; p : 0.10). In our study, the sensitivityof MRI in detection of spondyloarthrites lesions was64.3% at the sacroiliac joints and 54.3 at lumbar spine.Conclusion: MRI allowed to diagnose sacroiliitisin 46.2% of patients in preradiographic stage. MRIseems less sensitive than standard x-rays in detectingsacroiliitis in advanced forms of the spondyloarthritis.
