Abstract :
Introduction : Primary hyperparathyroidism (HPTp) is rare in Africa. We report 22 observations. Materials and methods: This is a retrospective study in the Rheumatology’s service of the Aristide Le Dantec Hospital of Dakar. We collected observations of Primary hyperparathyroidism between January 2000 and June 2017. The diagnosis was based on the association of permanent hypercalcemia and a serum increase in parathormone 1-84 assayed by immunoassay. Epidemiological, diagnostic and therapeutic data were analyzed for each patient. Results: Twenty-two observations were recorded in 18 women and 4 men, with an average age of 51.35 years. Associated pathology were 2 cases of multiple neuroendocrine neoplasia type II, and systemic diseases in 8 patients. The phosphocalcic anomalies had an average hypercalcaemia of 117.70 mg / l, a hypophosphatemia at 30.04 mg / l, an increase 24-hour phosphaturia on average to 342.7 mg/l. Serum parathormone 1-84 was increased with an average rate of 248.21 ng / l. The parahyroid lesions were hyperplasia in 3 cases and an adenoma in 19 cases. The evolution was favorable except in 2 patients who died of co-morbidities. Conclusion : We propose a systematic dosage of serum calcium especially in women over 50 years of age, for early detection at the asymptomatic stage of primary hyperparathyroidism.
