An Interesting Complication of Temporomandibular Joint Arthrocentesis: Case Report
Abstract :
This case report aims to review the technical details of arthrocentesis and report an unusual complication seen in a 22 years old lady. The patient received arthrocentesis in an attempt to treat painful locking episodes on her left temporomandibular joint (TMJ). During the procedure intrarticular lidocain injection was given to eliminate pain and feeling of pressure. Anaesthesia of the ipsilateral lingual and inferior alveolar nerves was observed after the procedure that lasted for an hour. No persistent complications were noted during the post-operative reviews. It was thought that an inadvertent perforation on the medial capsular wall probably resulted in extravasation of saline and local anaesthetic to the infratemporal fossa leading to anaesthesia of the adjacent mandibular branch of the trigeminal nerve. According to our knowledge this complication of arthrocentesis has not been reported before. Limiting the extravasation of fluids during arhrocentesis is imperative to avoid undue anaesthesia or airway compromise.
