EFFECTS OF THE CHANGE IN THE METATARSAL LENGTH ON THE CLINICAL OUTCOME AFTER SURGICAL TREATMENT HALLUX VALGUS

Abstract
OBJECTIVE: The main purpose of the present studywas to evaluate to what degree the change inmetatarsal length can affect the clinical outcomes inpatients who underwent either a distal chevron or aproximal cres-centic metatarsal osteotomy for halluxvalgus.MATERIALS AND METHODS: Sixty-two patientswere included in this study. Nine of the patientswere male and 53 were fe-male. The mean age of thepatients during the surgical intervention was 45.9years. Thirty patients underwent crescentic proximalmetatarsal osteotomy and 32 un-derwent distalchevron metatarsal osteotomy as the treatment ofhallux valgus. The mean follow-up was 24.3months. Postoperatively, clinical as well as theradiographic evaluation was carried-out at the endof the forth week, third, sixth and twelfth months,and annually thereafter. The American OrthopedicFoot and Ankle Society forefoot (AOFAS) score, VisualAnalo-gue Scale (VAS), metatarsal length, halluxvalgus angle (HVA), and intermetatarsal angle (IMA)were evaluated.RESULTS: The mean HVA as well as the mean IMAimproved significantly in both group of patients. Themean change in the length of the first metatarsal was-2.8 mm in distal chevron group and -1.2 mm in crescenticproximal osteotomy group. The mean correctionof hallux valgus angle was 13.6 degrees in distalchevron osteotomy whereas it was 20.4 degrees increscentic proximal osteotomy. The patients with 3mm of shortening had significantly worse VAS scores.We did not observe any non-union or avascular necrosisof the metatarsal head. At the latest follow-up, 8patients from DCMO group and 5 patients from CPMOgroup mentioned that they would not recommendsurgery to others with similar complaints.CONCLUSION: Distal chevron osteotomy leadshorte-ning of the first metatarsal bone resulting intransfer metatarsalgia and worse pain scores morecommonly than crescentic proximal metatarsal osteotomy.
