Abstract :
An analysis of human errors in nuclear power plant systems when measured against common-mode failures (CMF) is presented. Human errors evaluated are improper testing, inadequate maintenance strategy, and miscalibration. The methodology improves power plant systems availability by identifying sources of common-mode failure when operational functions are involved. It is also applicable to other complex systems such as chemical plants, aircraft, and motor industries; in fact, any large man-created, man-machine system could be included
