Abstract :
In large, complex systems, even the most careful designer can lose sight of an obvious error. The examples cited are: a well-tested electronic publishing system that suddenly fell into erratic behaviour, hermetic packaging breakdowns, and how designers of a commuter train system (BART) failed to compensate for one of the most obvious of all reliability problems: stray current leakage that would drain traction power as well as pose a shock hazard to passengers.
