Abstract
Human inappropriate actions at Virginia Power nuclear plants are identified, analyzed, and reported to management for corrective action. The program that is utilized to accomplish this effort is the Institute of Nuclear Power Operations (INPO)-sponsored human performance evaluation system (HPES). The author reports on the comprehensive methods that have been used to implement and institutionalize the HPES program at Virginia Power to identify root causes of human inappropriate actions and reduce the adverse effects of such actions.<>
