    This paper aims to find the cause leading to aviation accident and put forward relevant prevention strategy from organizational level. 7 types of direct reason causing aviation accidents are analyzed and classifies by using method of System Theory and `Human-Machine-Environment´ Model which was proposed out in previous. Then a Stat. on 21 Chinese civil aviation accidents happened from 1981 to 2008 is made and get a conclusion that crew factors had the greatest contribution (61.9%) on aviation accidents and prevention measures should be deployed from organizational level. Then prevention strategies are proposed out according to analysis results and case analysis is given meanwhile. Finally it concluded that education and training, stimulating system, rules and procedures, culture climate and supervision are key factors to control crew errors and aviation accidents on organizational level.
