Abstract
The utility of a medical records system is related to the ease of data and information entry and the usefulness and functionality of the data and information representation. We present a brief review of traditional and graphical entry and representation methods. We then present developments in the Visual Chart, a computer-based medical record interface containing time- and problem-oriented views based on iconic representations of medical events. The Visual Chart uses many graphical entry and representation methods and may provide a more intuitive and useful way of interacting with medical data and information.<>
