A database system for form data and form description data management

On the Web, the use of online forms to gather user input is constantly increasing. To simplify the task of generating and handling such forms, we propose the creation of a database describing the layout and content of such forms. In this paper, tables describing form data are defined, and constraints among the attributes of each are discussed to avoid form errors. In addition, the types of errors that may be generated were considered based on an example form created for a helpful Incident Report System at Kyushu University Hospital. We examined how restrictions can be applied to the table definitions for errors and considered methods of avoiding them.
