Web-Based Coordination Support in Care Planning

Starting from a case study in the area of health care, the paper introduces a Web-based coordination support application in care planning. Main contributions to research in the field of Web computing and CSCW are (1) an experience report on how to shape Web-based coordination support for care planning and (2) a discussion of how to apply concepts from the areas communities of practice and coordination mechanisms to the field of care planning. Analysis of facilities useful for communities of practice led to a number of proposals for functional enhancement of the developed application in the field of knowledge sharing.
