Abstract :
This paper discusses lessons learned in the nuclear industry following the Three Mile Island accident and how they maybe used to improve the petrochemical industry safety. The process of improvement is discussed in the context of the British Petroleum Texas City accident. The paper refers to PRA/HRA methods and if applied to the Petrochemical industry could help management in its decision-making to improve both safety and availability of refinery operations. 15 persons were killed and 150 were injured in the BP accident.
