MyCareTeam: a disease management Web application

There has been an explosion of medical information and content on the World Wide Web in the past few years. More and more individuals use the WWW to find out about a disease or illness, to learn about possible treatments and cures, or to learn about new research on a given illness. MyCareTeam is an interactive Web application that utilizes the Internet and computer technologies to provide interactive, customized care to patients with diabetes. It allows individuals with diabetes to better monitor their glucose levels, understand their lab values, realize the effect exercise and diet have on their disease, get educational information and most importantly to better communicate with their healthcare team on these issues
