Retrograde progress of reliability analysis methods

Abstract :
Several examples are presented to show how some development of reliability analysis methods has led to a resurgence of mistakes and pitfalls, which were believed to have been resolved a long time ago. A review of those cases indicates that the major cause of this undesirable outcome is the emphasis on procedures, while overlooking the importance of definitions and basic concepts, including the statement of attached limitations. A change in the attitude towards the teaching, development and application of reliability analysis methods will be needed to prevent serious problems.
