  • Abstract
Alice in wonderland syndrome is associated with migraine, epilepsy, infectious mononucleosis, Epstein-Barre viral infection and psychological disorders. It has rarely been reported in children. We report one case of pediatric migraine with onset as AIWS. A 13 years-old boy who experienced paroxysmal visual illusions for 6 years and headaches for 1 year. The child was previously misdiagnosed as pediatric schizophrenia. Symptoms of throbbing headache developed eventually. He had a family history of migraine Physical examination was unremarkable. Laboratory data such as brain CT, VEEG, routine test of cerebrospinal fluid, routine blood test, atypical lymphocyte in the peripheral blood smear and antibodies to EB virus were normal. TCD revealed the increased blood stream velocity The clinical diagnosis was made and topiramate was administrated. The symptoms of AIWS and headache resolved over half an year. Alertness to AIWS is helpful for making proper diagnosis and appropriate treatment of its associated disorders.
