Challenging the blame culture: a case study of a mistakes workshop

This paper describes a training workshop on mistakes conducted for Open University staff in June 1995 as part of a programme to encourage grass-roots staff involvement in organisational change at the University. The workshop is one way in which the organisation hopes to foster shared learning from mistakes and so is trying to reduce recourse to blame
