Abstract
Summary form only given. In this work, we fabricated a mirror with a nonaxisymmetric surface profile by plasma chemical vaporization machining (CVM) for laser beam forming of a Gaussian beam into a rectangular uniform-intensity beam. The nonaxisymmetric mirrors were fabricated from square plates or circular disks made of silica glass. The fabricated area was rectangular with the width of 12 mm and the maximum depth of about 3 /spl mu/m. The mirror was designed to transform an input Gaussian beam with the diameter of 6 mm into a rectangular beam with the width of 6 mm at 1.4 m from the mirror.
